Chironomidae is enormous — one of the most widely distributed insect families on earth, and in a lot of water it is the majority of the available biomass by count. It is also the group most often written off, because everything about it is small.
The larva is the giveaway: a soft, segmented, worm-like body with a distinct hardened head capsule, a pair of prolegs under the head and another pair at the tail, and no segmented true legs anywhere. Collected in a tray it usually curls into a C or J. The blood-red forms people notice are haemoglobin-bearing larvae from low-oxygen sediment; colour is common but not diagnostic.
The pupa rises to the film and hangs there vertically, thorax up, with a pale gill tuft at the head — often the single most-eaten form on a hard winter day. Adults are delicate, two-winged, and cluster in the air and on the film in numbers.

Mayfly nymphs have three pairs of jointed legs, long tails, and abdominal gills. Midge larvae have none of those — only prolegs.
A midge larva is a worm of even thickness, free-living or in a soft tube, and it is not holding on to anything. A black fly larva has a swollen rear end gripping the rock, so it stands up off the stone and waves downstream — usually with a colony of them doing the same.
These groups name Midge (non-biting) in their own separators. If you arrived holding one of them and were not sure, this is the sentence that pointed you over.

A hard head capsule and prolegs at each end is a midge larva, which is an insect. A leech has neither.
A hard dark head clearly out in front, a thin worm-like body and two pairs of hook-tipped prolegs is a midge. Bright red points that way too. Size and habitat separate nothing here.
