First checkSize first. If it is under 25 mm you are almost certainly not looking at a mature hellgrammite. Then check for the row of lateral filaments down both sides.
The hellgrammite is the aquatic larva of a dobsonfly, and it is the largest thing most people will turn over a rock and find. Mature larvae routinely run 40–80 mm and can exceed 100 mm. Nothing else in cold flowing water looks quite like it once you have seen one.
The diagnostic combination is size plus lateral filaments plus mandibles: eight pairs of filamentous appendages ranked down the sides of the abdomen, usually with a gill tuft at each base, three pairs of thoracic legs, a terminal pair of clawed prolegs, and a heavy head carrying mandibles strong enough to draw blood.
It wants well-oxygenated flowing water and lives under stones in riffles and runs. The winged adult is terrestrial and sits outside this aquatic-larva reference entirely.

A hellgrammite finishes in a pair of hooked prolegs and carries eight pairs of lateral filaments with cottony gill tufts at the base; an alderfly finishes in a single filament pointing straight back, with seven pairs and no tufts. The alderfly is also smaller, paler and in soft silt rather than fast cobble.

The larval stage runs one to several years, so hellgrammites of every size are in the stream continuously. The seasonal event is not the food item appearing — it is the biggest ones leaving to pupate on land, fairly synchronously, sometimes after a thunderstorm.
